A MyMelanomaTeam Member asked a question 💭
Seward, AK

Brian has Stage 3b metastatic melanoma with the BRAF mutation, diagnosed August of 2023. Keytruda and the Ipi/Nivo infusions were not effective. His cancer has spread from superficial lesions on his scalp (both removed with clear margins) to the liver. Since August of 2024 it has now spread extensively in the liver as well as to both lungs, an adrenal gland, soft tissue, and several bones. He is an otherwise very healthy 52 year old, tolerates the treatment well but his cancer is still… read more

Jennifer and Brian, please study the heck out of Dr. Seyfried. Sooner rather than later. Start with his YouTubes online, then get his main book, or at least some of the smaller/thinner ones that have his info. Target semi-starving of the cancer cells to the maximum extent possible. Just on your own, target a GKI of 1 to 2 to slow those tumors down. I check mine every morn and would encourage you to.

I'm sorry to hear about Brian's situation. Here is some information about Mektovi (binimetinib) and Braftovi (encorafenib) based on my knowledge base:

Mektovi (Binimetinib)
- Class: Kinase inhibitor
- Mechanism: Inhibits MEK1 and MEK2 proteins, slowing cancer cell growth.
- Administration: Taken orally, twice daily, along with encorafenib.
- Common Side Effects: Fatigue, nausea, diarrhea, vomiting, abdominal pain.
- Serious Side Effects: New primary cancers, cardiomyopathy, blood clots, eye problems, lung scarring, liver damage, muscle problems, hemorrhage, fetal harm.

Braftovi (Encorafenib)
- Class: Kinase inhibitor
- Mechanism: Inhibits BRAF protein, targeting BRAF V600E receptors to slow cancer progression.
- Administration: Taken orally, once daily, along with binimetinib.
- Common Side Effects: Fatigue, nausea, vomiting, abdominal pain, joint pain.
- Serious Side Effects: New primary cancers, increased cell numbers in BRAF wild-type tumors, hemorrhage, eye inflammation, QT prolongation, fetal harm.

General Notes
- Combination Therapy: Mektovi and Braftovi are often used together to treat unresectable or metastatic melanoma with BRAF V600E or V600K mutations.
- Monitoring: Regular monitoring for side effects and effectiveness is crucial.

Given Brian's extensive spread of melanoma and previous treatments, it's important to closely monitor his response to Mektovi and Braftovi and report any side effects to his healthcare team.
